Associated Health Trainees on Stipend

I. Application Forms/Training

All forms/certificates that you complete in A. and B. need to be returned with your packet. It will take approximately 2 hours to complete the forms and training. A list of required forms to submit, the recommended timing, and individual responsible for initiating the steps is included in the Associated Health - Paid Checklist. Please include this checklist with your packet.

A. Forms

Make sure you have a printer available, as some of these forms cannot be saved.  Please complete and print all of these forms.  Retain a copy of all completed forms for your records.

B. Mandatory Training

This training is designed for residents/trainees who are new to the facility and have never completed the mandatory training modules. The course is titled "VHA Mandatory Training for Trainees."   Print and follow the TMS instructions for self registration then log on to the website below, complete the training, and print the certificate.

Be sure to include a copy of your mandatory training certificate of completion in your application packet provided to your service secretary before you rotate at the Birmingham VAMC.

Please mail completed forms and training certificates to your designated service contact at the address indicated below within the timeframe indicated on your checklist. Be sure to include the appropriate routing number for your service area. D. Fingerprinting and Photo IDs

Directions for Fingerprinting and Photo IDs

Contact Personnel Security employees [Caleb Turner, VA ext. 4516, Linda Walfield, VA ext. 5958 or Doug Lilly, VA ext. 5817] to schedule an appointment for fingerprinting and photo identification. Fingerprinting must be completed in a timely manner to ensure processing before the start of your training program. Personnel Security is located in Room 2208 of the Birmingham VA Medical Center.

E. Orientation

You will need to bring two forms of identification for fingerprinting/photo identification, and orientation:

  • One government issued photo ID (i.e., driver license, military ID, passport)
  • One non-photo identification (i.e., Social Security Card, certified birth certificate, state voter registration card, Certificate of U.S. Citizenship or Certificate of Naturalization)
  • Permanent Resident Card (if applicable).

II. Requirements at the Conclusion of VA Training

For residents completing a program and leaving the VA:

  • Verify that you have completed and signed all progress notes, orders, and patient requests.
  • Turn in your VA ID badge to the same location where you obtained it.
  • Check out with your service or department.
  • Computer Clearance.
  • Complete the Learners' Perception Survey.
  • Consider VA Employment. (See Section V on home page.)

Patient Privacy: Patient privacy at the BVAMC is not only governed by the HIPAA regulations, but also the Privacy Act. Patient information may not be taken out of the BVAMC building in paper or electronic form under any circumstances.
